To apply for a place at Midstream please contact our Training & Operations Manager or our Admissions Administrator. You will be sent an Application Form and a Safeguarding Questionnaire/Declaration. You will be asked to provide details of other professionals who we can contact to also complete a Safeguarding Questionnaire/Declaration.
